Important Notice As you know, the Registry works to update all of our donors' files every three years. It is extremely important that we have updated contact information for you on file here at the Registry. The most important reason for updating your donor file is to be sure we have your most up-to-date otologic records. This greatly increases the value of your donation. Please take a minute to look over the list below and see if any of the following pertain to you.
Your address has changed in the last three years Please be sure to fill out both of the forms located at the bottom of the page and forward them to the Registry if in the last three years: 1) You have married and your name has changed 2) Your next of kin information has changed (the designated person, their address or their phone number) 3) You have visited an ear, nose and throat physician 4) You have visited an audiologist 5) You have been tested for a hearing aid 6) You have had balance tests performed 7) You have had ear surgery 8) You have had an MRI or CT Scan of your temporal bone 9) You have received treatment for an otologic disorder 10) You have not previously sent your updated forms back to the Registry The forms can be forwarded via mail, fax or email:
